Send a request without an account

Choose Privacy or data removal in the Contact us form. You do not need to buy a list or register an account.

Include your name and enough information to locate the record, such as NPI, specialty, practice location or the email concerned. Say whether you want access, correction, deletion or exclusion from future sale or sharing where applicable.

Do not send government ID documents, patient information, passwords or full payment credentials in the initial request.

What happens next

The form submits a request for review; it does not instantly change a record or confirm removal. We may need clarification to locate the correct information. Appropriate identity checks can apply to requests involving private account information.

The outcome can depend on applicable law, retention duties and information previously disclosed to purchasers. Applicable legal deadlines and rights are not replaced by this page.

Different types of opt-out

Browser cookie preferences do not remove a professional record from a contact list. Unsubscribing from a message and requesting exclusion from list sales are also different actions. Tell us which request you are making.

Agents and regional rights

An authorized agent may submit a request; explain the relationship so we can identify appropriate authorization checks. California and EEA notices describe regional rights.

You will not be unlawfully penalized for exercising a privacy right.
